Once all requests have been received, they will be evaluated based on the following, ordered criteria:
Individuals will be notified on or before June 5, 2006 in regards to their locker assignment. Payment must be made by June 21, 2006 or the hold will be canceled. All requests submitted after May 31, 2006 will be handled on a first come, first served basis. All locker rentals are effective July 1, 2006. Please note - by completing this form, you are indicating that you wish to rent a locker at the Wellness Center - once the preferences are sorted a bill will be issued. Locker Rental Options (Please indicate your first and second option.) -- 1st Preference -- Premium Locker - 12 months / $120.00 Standard Locker - 12 month - $60.00 Standard Locker - 4 month / $25.00 -- 2nd Preference -- Standard Locker - 12 month / $60.00 Standard Locker - 4 Month / $25.00 Premium Locker Rental (Men’s) – ($120.00) – SOLD OUT - Waiting List Only!!! A tall locker or Z-style locker located inside the Men’s locker room. This locker is a 12-month renewable rental. There are 18 lockers available in the Premium Men’s section.
Premium Locker Rental (Women’s) – ($120.00) – SOLD OUT - Waiting List Only!!! A tall locker located inside the Women’s locker room, or a Z-locker located in the access hall outside the Women’s locker room. This locker is a 12-month renewable rental. There are 18 lockers available in the Premium Women’s section.
12-Month Locker Rental – ($60.00) A Z-style locker located immediately outside the locker rooms. 100 Z-style lockers are available for rental located immediately outside the locker rooms.
4-Month Locker Rental – ($25.00) A Z-style locker located immediately outside the locker rooms. 100 Z-style lockers are available for rental located immediately outside the locker rooms. Waiting List (Premium or Next Available) Individuals can note if they would like to be placed on a waiting list for a Premium Locker. The waiting list will be determined by items #2 and #3 listed above. The Premium waiting list is only open to individuals holding a current locker rental. Individuals will be required to pay the pro-rated difference when upgrading to a premium locker.

In the event all available lockers are rented, additional requests will be placed on a waiting list according to the date of application. Individuals will be notified once a locker is available. Any break in membership will result in removal from a waiting list.
Payment Options and Additional Guidelines All locker fees must be paid in full. Payment must be received within 14 days of notice from the Wellness Center or the hold will be canceled. Locker fees cannot be payroll deducted or charged on a monthly basis. All lockers must be secured with a personal lock. Locker fees that are 7 days overdue will result in the lock being removed and the contents held for 30 days. Contents not claimed within 30 days will be donated to a charitable organization. Daily-use lockers will continue to be available on a first come, first served basis. Payment of locker fees are non-transferable and non-refundable regardless of membership cancellation, termination or for any other reason. Multiple submissions will be disregarded.
