Application Process
- Download and print off a copy of the
Membership Application and the Payment Authorization Form.
- Read the corresponding Membership Terms
and Conditions and agree to abide to the requirements by signing
the bottom of the Membership Application. If applying for a Family membership, the
signatures of the member and their spouse are
required along with the names and birthdates of the dependent
members.
- All individuals who sign up for a
perpetual (monthly) membership must sign and complete the Payment
Authorization Form indicating either payroll deduction or
credit/debit card billing. This form is also used for individuals
desiring to use a credit or debit card for the payment of a
standard membership. Individuals paying for a standard membership
by check do not need to complete this form.
- Payment can be made in person at the
Wellness Center control desk 6 a.m. to 4 p.m., Monday-Friday.
