Application Process

  1. Download and print off a copy of the Membership Application and the Payment Authorization Form.
     
  2. Read the corresponding Membership Terms and Conditions and agree to abide to the requirements by signing the bottom of the Membership Application. If applying for a Family membership, the signatures of the member and their spouse are required along with the names and birthdates of the dependent members.
     
  3. All individuals who sign up for a perpetual (monthly) membership must sign and complete the Payment Authorization Form indicating either payroll deduction or credit/debit card billing. This form is also used for individuals desiring to use a credit or debit card for the payment of a standard membership. Individuals paying for a standard membership by check do not need to complete this form.
     
  4. Payment can be made in person at the Wellness Center control desk 6 a.m. to 4 p.m., Monday-Friday. Application forms and payment can be mailed to:
