Cardiac Rehabilitation: Daily Activity Journal

In order to provide our patients with all of the resources they need for successful cardiac rehabilitation, we have put together Cardiac Rehabilitation: Daily Activity Journal. This journal serves as a guide for the various activities necessary for rehab and a way to track you progress.
