  3. Criminal Background Check
      You are required to have a Criminal Background check completed. The Sheriff's Office in Augusta can provide this service for you for a $10.00 fee.  You will need to bring your social security card with you. They are located at 401 Walton Way. Their phone number is 706-821-1000.  You can also purchase one off of the internet. Some I have found by running a search are:

  4. Ten-panel Drug Screen
        You are required to have a ten-panel drug screen which includes amphetamines, cocaine, marijuana, opiates, phencyclidine, benzodiazapine, barbituates, propoxyphene, oxycodone/oxymorphone and merperidine. 
        If you have student insurance, Student Health can provide this for you at no charge through Mullins Lab.  When you go to Student Health, they will provide you with an order that you take to Mullins Lab. It will take 4-5 business days for your results to come back.
        If you get this done at Dekalb Medical Center, it costs $30 and takes 24 hours to have the results.
        You will not be able to begin your rotation until your site has this information.

Mileage Reimbursement:
SPCC Atlanta AHEC offers mileage support to students doing clinical rotations in Fulton, DeKalb, and Clayton counties.  In order to process this support, please complete the following two forms and submit them to the SPCC Atlanta AHEC Preceptor Coordinator. The contact information is listed below.
