Log cases weekly into SPEL by 8:00am Wednesday mornings weeks 2-6

 Case and Attendance record Card (green card)  due at MiniOSCE

Mid-rotation Feedback  due 3^rd Friday

Each logging assignment must be completed an outlined in the clerkship manual.

Please note: Final grades will not be issued until these tasks are completed even if no credit is given due to failure to meet deadlines

Follow the guidelines in the assignment description found in the Clerkship Manual.

Each packet should be stapled together in the order listed. The reflective piece should be completed on a Word processor unless it is graphic in nature.

H&P/DDX Assignments

Failure to complete the Differential Diagnosis component as outlined will result in no credit.

Failure to document a complete history, review of systems, breast and pelvic exam as outlined in the Clerkship manual will result in no credit. These are not SOAP type assignments
