Check In: All WellStar students must complete and fax the WellStar Confidentiality Statement form, Well Student Information form, Immunization Record and HIPPA Awareness Training printout to Janet Beard at (770) 792-1465 at least ONE WEEK PRIOR to the start of your rotation. The Student Information form must have a verification signature (the MCG Clerkship Coordinator can verify), and the Confidentiality Statement form must have a witness signature. You will not be able to start your rotation until this documentation is submitted and approved by Janet Beard.
Complete the WellStar Badge Application Worksheet and take it to Security Services at WellStar Kennestone Hospital Emergency Room or WellStar Cobb Hospital Emergency Room to obtain your WellStar badge. Please contact Janet Beard at (770) 792-5333 should you have any questions regarding the WellStar student forms.

You are expected at your preceptor's office by 9:00 AM the next day. You will
need to obtain your WellStar badge on the afternoon you arrive or before
reporting to your preceptor's office the next day.

Take Intestate 75 north to Exit 267 A. Make
sure you take the exit to Canton/Hwy. 5. Turn right on the exit to the
first traffic light. Turn right at the first traffic light at Sandy Plains
Road. Immediately get in the left lane. Turn left into the
Wellstar Corporate Office parking lot. Enter through the front door. As for Janet Beard, Clinical Coordinator.
