Welcome to Southwest Georgia!  We’re delighted to have you in a clinical site in our part of the world and hope that everything is going well for you as you settle into a new routine.  We’re anxious to help and assist you as needed.  Please let us know if we may be of service.

You are to report to Surgical Associates of Albany, P.C. (425 West Third Avenue, Suite 400) at 8:30 am on your first morning to meet your preceptor, Dr. Harry Vildibill, Jr.  Please ask for Dawn Thompson.

Also on your first day, please come to the SOWEGA-AHEC office (1512 W. Third Avenue) to:  a) complete our paperwork; b) complete hospital forms; and c) submit travel reimbursement forms.  We open at 8:00 am, and it will only take 15 minutes to complete the necessary forms. Please bring your immunization records with you.

You will also need to go to the Phoebe Putney Security Office located at 910 North Jefferson (in the parking deck behind the Emergency Center) to get your hospital badge (please see Keysha Wright).  Badges are issued Monday – Friday, 8:00 A.M. – 4:30 P.M.

                               

You are required to complete a JCAHO Session of Orientation for Phoebe Putney Memorial Hospital (please contact Ginette Mills to schedule orientation).  JCAHO TRAINING IS MANDATORY.  If you do not complete the training, you will not be allowed to complete your clinical rotation. 

Check Out:  At the end of your rotation, you are to return your keys and badge to the SOWEGA-AHEC Office. Please call if that is a problem. Grades will be held until all items are returned.
