Application Packet:

The candidate’s application must include the five components below.  

1.  Personal Statement:  The one page Personal Statement should include discussion of the candidate’s:

  • Educational philosophy
  • Professional development in education
  • Anticipated contributions to the goals of The Academy

2.  Educator’s Portfolio (EP):  The candidate should choose AT LEAST TWO, BUT NO MORE THAN THREE of the categories below to highlight in their EP.  (You may excel in more than three but must select which two or three to highlight in EP.)  The EP should contain documentation of quality and quantity for each category of educational activity supporting his/her nomination and should be no more than 9 pages.

  • Direct Teaching (all applicants must include this category)   See example

Supporting documentation should be provided to substantiate the quality or impact of the contributions in each category delineated in the EP. The EP should be submitted in accordance with published MCG guidelines.  The following may be helpful:

3.  A letter of support from the candidate’s department chair should be requested by the applicant. 

4.  A current Curriculum Vitae should be submitted in accordance with published MCG guidelines.  Curriculum Vitae (in MCG format)

5.  The MCG Academy of Medical Educators Membership Application Checklist should accompany each application.
