Please Note:
- The MCG Certificate of Immunization form listed at the bottom of this page is required for all new students attending classes upon acceptance to The Medical College of Georgia.
- Please print your form(s) from the links below. As a requirement for admission, complete the form(s) and return them to Student Health no later than 30 days prior to registration.
- All new MCG students have a hold on their account until immunization requirements have been met.To have the hold released, have the required Certificate of Immunization, along with required immune titer laboratory results, completed in English, signed by a certified health care provider, and submitted to MCG Student Health.
- If you fail to complete these requirements, you may be unable to attend classes.
- Please make a copy of
immunization form for future use.
