Please Note:
- Each of the six forms listed at the bottom of this page are required for all new students attending classes on the MCG campus upon acceptance to The Medical College of Georgia. If you are an online or distance education student who will not be attending classes on the Medical College of Georgia campus, you will only need to complete the Privacy Notice and the required Certificate of Immunization forms.
- Please print your forms from the links below. As a requirement for admission, complete the forms and return them to us no later than 30 days prior to registration.
- If you choose to submit documentation other than the required certificate of immunization form, you must submit a payment of $100.00 with the immunization information to cover the extra administrative time required to review the documents. If you do not submit payment with the documents, your immunization record will not be considered complete and you will not be allowed to enroll.
- Please make a copy of immunization form for future use.
- THESE FORMS MUST BE RETURNED TO STUDENT HEALTH SERVICE THIRTY (30) DAYS PRIOR TO YOUR REGISTRATION DATE.
- FAILURE TO SUBMIT THE REQUESTED MEDICAL INFORMATION AND DOCUMENTATION OF IMMUNIZATIONS WILL RESULT IN THE INABILITY TO REGISTER FOR CLASSES.
