Authorization For Release Of Education Records


The Family Educational Rights and Privacy Act (FERPA) affords certain rights to students concerning the privacy of, and access to, their education records.  Students may complete and submit this form to the Office of the Registrar to allow the release of designated education records to specified third parties.

Completed forms should be submitted to the Office of the Registrar, Medical College of Georgia, AA-173, Augusta, Georgia 30912-7315, or faxed to 706-721-0186.
