Confidential Disclosure Forms

A Confidential Disclosure Form (“CDA”) should be in place whenever an MCG researcher will be divulging information that is not yet in the pubic domain. This is especially important to remember when potential patent applications are being considered or planned. Without such a form in place, the disclosure of proprietary information will start a one-year clock ticking. After its expiration, we have lost the ability to file for a U.S. patent.

The form has to be signed by an appropriate MCG representative
prior to the disclosure of any proprietary information. The signature of a faculty researcher is not sufficient and the document is not legally binding.

The CDA form is prepared in the Office of Technology Transfer and Economic Development. The following information is necessary to complete the form and should be forwarded via email to jukelley@mcg.edu, or by telephone to Julie Kelley @ 721-4122.

v     Faculty members name, department, campus address and contact phone number

v     Information to be disclosed

v     Name, university or company, and mailing address of the other party

You will be notified once the CDA has been signed by the appropriate MCG representative and is ready to be picked up.  There will be two originals to be mailed to the other party.  Their authorized representative is to sign both and return one fully executed original to our office.
