H-1B Checklist Effective June 2005 To begin the H-1B petition process: review the checklist, complete the appropriate forms, and provide the necessary documents to the International Postdoctoral Office at: Medical College of Georgia International & Postdoctoral Services (IPSO) 1120 15th Street, CJ 2211 Augusta, GA 30912 Please be aware that every case varies and different document may be required for each individual petition.
1) From the Hiring Department An H-1B Application consists of the following documents, submitted from both the Hiring Department and the H-1B applicant. □ Completed H-1B Department Request Form from: Department Chair, Institute or Center Director mailed or faxed to IPSO office □ Copy of job-offer letter for new hires. For Postdoctoral Fellow positions, please see the Postdoctoral Guidelines for information on how to obtain the template offer letter for postdocs. For classified positions, we will need a copy of the official offer letter generated by Human Resources. For faculty appointments, we will need a copy of the official appointment letter. □ USCIS Filing Fees of $320.00 and $500.00 Two separate checks in the amounts of $320.00 and $500.00 payable to: Department of Homeland Security, U.S. Citizenship and Immigration Services, California Service Center, ATTN: CAP EXEMPT H-1B Processing Unit P. 2) From the Applicant/Employee
Applicants currently in the United States, not holding an H-1B through MCG □ H-1B Applicant Form (PDF form) □ Copy of current I-94 (front and back) □ Copy of appropriate work permission document such as: current I-20, EAD, IAP-66/DS-2019 & evidence of 212(e) Waiver (Department of State letter OR I-612 INS Approval Notice) □ Copy of passport (e.g. main information page, visa stamps, passport expiration date) □ Copies of all academic degrees, licenses, and/or certifications (Foreign documents must be translated to the English language). □ A document-by-document credential evaluation from American company. Note: Applicants currently holding an

documents listed above (except credential evaluation) including their current H-1B Approval Notice (Form I-797)

4) IPSO will □ Conduct Prevailing Wage Analysis with the Georgia Department of Labor. (Note: The salary offered must meet or exceed either the current Prevailing Wage as determined by the Department of Labor, or the Actual Wage as determined by IPSO, whichever is the higher value.) □ Draft Support Letter to be signed by Department Chair, Institution or Center Director. The Support Letter will include the summary of the position being offered as stated in the Department H-1B Request Form and the description of why the individual uniquely meets the qualifications for the position offered. □ Draft Job-offer letter for H-1 extension, or change of status J-1 to H-1, to be signed by the applicant's supervisor. □ Complete USCIS Forms, provide institutional signatures, and mail completed package to USCIS.
