This disclosure form is meant to be used by MCG faculty, staff, and students in complying with the Medical College of Georgia Intellectual Property Policy. Reference to the policy should be made for clarification regarding definitions or applications.

The form is available as IPDForm.rtf (Rich Text Format file) that can be downloaded and opened in a word processing program such as Word Perfect or Microsoft Word or as IPDForm.pdf (Portable Document Format file) that can be printed and filled out.
