Research Note Templates

The "Research Note" templates are available for individuals conducting clinical research.  The scripts have been typed onto the approved form with italicized instructions for additions. 

The format of these "Research Note" templates cannot be changed.  The information provided must not be altered and the margins cannot be changed.  The appropriate text from the sample scripts may be added to the forms and personalized for any research study.  

The bar code for research was added to the bottom of each template. Note:  There are two (2) types of bar codes.  The "research" bar code is to be applied to the "Research Note" templates since they are visit-specific.  The "study" barcode is to be added to each page of the informed consent document and to the Research Alert.
