These templates have been approved by the Human Assurance Committee (HAC). Use of applicable templates is highly recommended since the templates can be used as a guide, and customized for each protocol.

How to Use the Templates:
To use the templates, highlight the text, copy into a word-processing file, and insert appropriate information into italicized areas.  Any italicized statements are guidelines only and should be deleted prior to HAC submission.
