Guidelines to use if you are in the School of Medicine and/or are using MCGHI Resources (e.g. staff, equipment, patients, patient records, facilities)
STEP 1: Before beginning the submission process, everyone on the research team at MCG must complete the required research education, CITI (click here for CITI registration instructions).
STEP 2: Submit the required documents to the MCG HealthSystem OCIS Review Office and completed their submission process (Their submission process will also involves Steps 3-6 listed below).
STEP 3: Complete the appropriate HAC forms (click here to access the HAC Forms web page)
*HAC Forms 100 and 101 are required for all protocol submissions
STEP 4: Create a Description of Research Proposal (click here to access the requirements for creating a description of research proposal)
STEP 5: Create a Research Informed Consent Document, if applicable
STEP 6: Ensure that all required signatures have been obtained on HAC forms being submitted
