Before
ANY researcher can obtain a federal DEA license, they must obtain a
Georgia DEA license. The current cost of the
Georgia DEA license application is $50. MCG researchers can waive the
federal DEA application fee because MCG is a state institution. The
Georgia DEA license application requires a photograph and notarized
signature. The research DEA application can be found in on the website of the Georgia

DEA Licenses
for MCG Faculty - Only
pharmacists, MD's DVMs, PI's, etc. should be registering in their own name
followed by MCG. (See example of form DEA-225)

The
Georgia research DEA applications typically require this information:
- A clearly stated purpose
for the project in layman terms.
- The correct schedule and
code number used for each drug.
- Identify the approximate
amount of drug used per year.
- Identify how the drugs
are to be handled and/or used for research purpose(s).
- Identify storage location
of drugs. Identify the
building, and when the building is open to the public and when locked.
Is there an alarm system (yes, no)?
Where will the drugs be obtained from?
Who will have keys to the locked storage place? Who
will have access to the controlled substances? If the drugs are locked in a desk, then this must be
over 750lbs in weight. Where
will the keys be stored?
- To provide detailed info
regarding who will use the drugs. The
access to drugs requires identification with the following (please note
that no one with a felony drug-related conviction may have access):
a. full name of each individual (including middle name)
- title
- date of birth
- race and sex
- nationality
- eye
color
- hair
color
- height
- weight
- resident
address
- social
security number
- telephone
- Has the
individual ever held a prior DEA registration?
- Georgia application
requires a current photograph.
- Georgia application
requires notarized signature.

Note:
The federal DEA application requires a state license number that may be
“pending.”
Note:
State requires 5 years of record documentation; federal requirements are 2
years.
For
documentation:
- Drug information must be
kept in a bound log book. If
changes are made to an entry, a single line should be drawn through it,
and initialed by the PI and dated and witnessed by a second person and
dated. No white-outs are
allowed.
- Log books should be kept
with locked-up controlled substances with keys kept in secure location.
- The
log book should show when drugs are received, how much was used on any
given date, etc. The log
book must show the complete input and output for each drug.
