Better Informed Patients Was the Goal of Weinrich’s Prostate Research
 

Dr. Sally Weinrich with patientThe Decision-Making for Prostate Cancer Study was funded by the American Cancer Society for four years: January 1, 2001 - December 31, 2004. A total of 361 men participated in the educational program. Over half, 56%, of the sample was low income. The average age was 53 years. Seventy-four percent of those men attending were African American. Fifty-eight percent reported completing some schooling beyond high school. The majority of participants were married, 58%. Almost 20% of the men reported having a family history of prostate cancer. About one-third of the men had previous history of PSA screening while over half of the men reported previous DRE screening.

The research tested the effect of two different brochure decision aids. The enhanced decision aid, Your Life - You Decide, based on Social Learning Theory, included supporting and opposing opinions from physicians and peers. The usual care decision aid, Should I Be Tested for Prostate Cancer? was developed by the American Cancer Society. Both decision aids presented the benefits and limitations of prostate cancer screening. The enhanced decision aid was associated with higher post-test knowledge scores, but statistically significant differences were observed only in the men who reported having had a previous DRE (p=0.013, t=2.52 in men with a DRE). All the men decided to be screened, regardless of which decision aid they received. This study highlights the impact of previous screening on education, and challenges the assumption that increased knowledge of the limitations of prostate cancer screening will lead to decreased screening. That all men decided to be screened is noteworthy for public health.

South Carolina Prostate Cancer Project

The South Carolina Prostate Cancer Project was funded February 1994 - March 1998 by the National Cancer Institutes of Health to test educational interventions aimed at encouraging at-risk men to go for prostate cancer screening. A summary follows:

  • 2,170 men attended prostate cancer educational programs at 220 different sites.
  • 1,494 men received prostate cancer screening.
  • 27 men were diagnosed with prostate cancer.
  • 24 of the prostate cancers were in the early stage and had not spread outside of the prostate gland!

Among men in the Prostate Cancer Project, occupations in the tire or rubber industry were associated with abnormal screening results for some of the men.

 

Dr. Sally Weinrich is Professor at the University of Louisville. Honors include the Order of the Palmetto from the governor of South Carolina and the American Cancer Society's Pioneer Award. Dr. Weinrich has had the privilege of working with African Americans in early detection of cancer for the last 18 years. Past research has included a NIH funded Prostate Cancer Project, an ACS institutional funded grant, and numerous small grants dealing with cancer detection.
