I hereby consent to allow ___________________________ (the
"Minor") to use the Greenblatt Library at the Medical College of
Georgia. I understand that the library’s collection includes legitimate
academic materials regarding anatomy and sexuality which some may find
offensive. In addition, I understand that library users may operate
computer workstations with access to the Internet. Although it is against
the law and MCG policy to use the Internet for any illegal purpose, such
as the distribution of obscene materials to minors, MCG and its library
staff do not monitor Internet usage, and MCG assumes no responsibility for
any use of its computer workstations. I acknowledge that MCG reserves the
right to revoke library privileges to those who fail to follow the rules
of the Greenblatt Library, and to remove such persons if necessary. I will
be responsible for any late fees imposed on Minor pursuant to the rules of
the Greenblatt Library and for any damage to the library or its collection
caused by Minor.
