All benefits-eligible employees receive basic life insurance coverage in the amount of $25,000 at no cost. Basic Life Insurance has an Accidental Death & Dismemberment (AD&D) provision.
Employees may purchase supplemental insurance in the amount of up to three times their annual salary (maximum coverage of $300,000) or a flat $15,000 policy. Supplemental Life Insurance also has an Accidental Death & Dismemberment (AD&D) provision.
Premiums per $1,000 of coverage are age-based. Employees age 67 or older at the time of employment, reductions in Supplemental Life will apply. Please contact the Benefits & Data Management Staff @ 721-3770 for additional information.
Dependent life insurance is also available to employees who are eligible for group life insurance. Dependent life insurance provides coverage in the amount of:
Premium is $5.81 per month for the entire family unit.
*It is the employee's responsibility to notify the Human Resources Benefits & Data Management Staff @ 721-3770 if a dependent age 19 or older is no longer eligible for dependent life coverage. Failure to contact Human Resources may result in non-refundable premiums being paid to the vendor.
Accidental death and dismemberment insurance is available to all benefits-eligible employees with a work commitment of .50 or greater.
