THE MEDICAL COLLEGE OF GEORGIA ADMINISTRATIVE PROCEDURES Office of Primary Responsibility - Human Resources - Benefits Subject: Long-Term Disability Enrollment Appendix E (pg. 4 of 10)
1.0 Purpose: To present the procedures for processing short and long-term disability enrollment.
2.0 Note: Long-term disability provides income in the event of your disability. There is a 180 day waiting period (6 months). Premiums are based on age and salary.
3.0 Procedure: Processing Short and Long-Term Disability Enrollment 3.1 Responsibility: Employee 3.1.1 Meets with Benefits Office during open enrollment to elect disability benefit. 3.2 Responsibility: Benefits Office 3.2.1 Explains disability information and prices. 3.2.2 Assists employee with completing appropriate election form. 3.2.3 Mails form for review to Metlife. 3.3 Responsibility: Metlife 3.3.1 Reviews form. 3.3.1.1 If approved, notifies Benefits Office of decision. 3.3.1.2 If not approved, notifies employee. 3.4 Responsibility: Benefits Office 3.4.1 If approved, Benefits enters into PeopleSoft. 3.4.2 Sends form to Records.
