THE MEDICAL COLLEGE OF GEORGIA ADMINISTRATIVE PROCEDURES Office of Primary Responsibility - Human Resources - Benefits Subject: Long-Term Disability Claims
1.0 Purpose: To present the procedures for processing a long-term disability claim (LTD).
2.0 Note: Long-term disability provides income in the event of your disability. There is a 180 day waiting period (6 months). Premiums are based on age and salary.
3.0 Procedure: Processing Long-Term Disability 3.1 Responsibility: Employee 3.1.1 Experiences an injury or illness. 3.1.2 Upon notification by a physician that your absence/illness/treatment will extend 6 months, immediately completes/obtains the following: Employee Statement, Attending Physician Statement, Employer Statement. 3.1.3 Turns forms/statements into Benefits Office. 3.2 Responsibility: Benefits Office 3.2.1 Receives employee and physician statements. 3.2.2 Reviews and ensures all forms are properly completed. 3.2.3 Completes the Employer Statement. 3.2.4 Obtains the employee’s job description. 3.2.5 Faxes all forms/statements to MetLife representative including: Employee Statement, Attending Physician Statement, Employer Statement, and Job Description. 3.3 Responsibility: MetLife 3.3.1 Reviews all documents and approves or disapproves the claim. 3.3.1.1 If approved, the paperwork is forwarded to the Benefits Office.
