THE MEDICAL COLLEGE OF GEORGIA ADMINISTRATIVE PROCEDURES Office of Primary Responsibility - Human Resources - Benefits Subject: AD&D Enrollment Appendix E (pg. 7 of 10)
1.0 Purpose: To present the procedures for enrolling an employee in Accidental Death and Dismemberment (AD&D) benefits.
1.1 Responsibility: Employee 1.1.1 Obtains AD&D form from the Benefits Office. 1.1.2 Completes and signs the form. 1.1.3 Submits form to the Benefits Office. 1.1 Responsibility: Benefits 1.1.1 Upon receipt of completed AD&D form, enters coverage into the PeopleSoft. 1.1.2 Gives employee a copy for their records. 1.1.3 Forwards AD&D form to Records for scanning.
