Subject: Short-Term Disability Enrollment Appendix E (pg. 3 of 10)

 

1.0   Purpose: To present the procedures for processing short-term disability (STD) enrollment.

 

2.0   Note: STD provides income in the event of your disability for up to 6 months.  Premiums are based on age and salary.

 

3.0   Procedure: Processing Short -Term Disability Enrollment

3.1   Responsibility: Employee

3.1.1         Initiates interest in STD on benefits enrollment form at the Benefits Office or during orientation.

3.2   Responsibility: Benefits Office

3.2.1         Faxes benefits enrollment form to AFLAC.

3.3   Responsibility: AFLAC

3.3.1         Sets up a meeting with employee to elect STD benefit.

3.3.2         Meets with employee and completes proper STD forms.

3.3.3         Submits a payroll authorization document to the Benefits Office.

3.4   Responsibility: Benefits Office

3.4.1         Submits authorization form to the Payroll Office.

3.5   Responsibility: Payroll

3.5.1         Processes in PeopleSoft.

3.5.2         Sends authorization form to Records for scanning.
