1.0 Purpose: To present the procedures for enrolling an employee in Accidental Death and Dismemberment (AD&D) benefits. 1.1 Responsibility: Employee 1.1.1 Notifies Benefits of any change in status or desire to enroll during open enrollment. 1.1.2 Completes AD&D form and submits to Benefits. 1.2 Responsibility: Benefits 1.2.1 Enters coverage into the PeopleSoft. 1.2.2 Gives employee a copy for their records. 1.2.3 Form is forwarded to Records for scanning. |
