Planned Shutdown and Equipment OutagesPart A - Vital Life Support Systems Procedure Anyone requiring service disruption must submit a SHUTDOWN REQUEST form to the Work Management Center at least five (5) business days prior to the requested shutdown date. Longer advance notice may be necessary for complex or involved shutdown requests. Shutdowns affecting patient care will be scheduled as conditions and safety permit. Note: Requestor must obtain prior approval from the Facilities Operation Manager for less than 5 business days advance notice. Part B - Non-vital Life Support Systems Procedure Anyone requiring service disruption must submit a SHUTDOWN REQUEST form to the Work Management Center at least 72 hours (3 business days) prior to the requested shutdown date. Longer advance notice may be necessary for complex or involved shutdown requests. Note: Requestor must obtain prior approval from the Facilities Operation Manager for less than 72 hours advance notice. Part C - Fire Alarm System Anyone requiring a fire alarm system disruption must submit a SHUTDOWN REQUEST form to the Work Management Center at least 48 hours (2 business days) prior to the requested shutdown date. Longer advance notice may be necessary for complex or involved shutdown requests. Note: Requestor must obtain prior approval from the Facilities Operation Manager for less than 48 hours advance notice.
