Maintenance Request Form

The following Maintenance Request Form may be used to submit maintenance related service requests to Facilities Management Division. This form should only be used for minor maintenance services.  

Please call in all emergencies and urgent requests to (706) 721-2434

Customers also have the option of calling in a work order at (706) 721-2434, faxing information to (706) 721-6501 or sending e-mail.
