MCG Express Card -
Terms and Conditions

Please read the following information carefully. It is the contract portion of this brochure. If you have any questions regarding the MCG ExpressCard, call the MCG ExpressCard office at (706) 721-9939 or write to:

The Medical College of Georgia hereby establishes an MCG ExpressCard account in the customer’s name for the amount receipted with the following conditions:

I. CONTRACT DURATION

The duration of these accounts shall be from the opening date of the account until one of the following occurs:

  1. Student graduation, dismissal or withdrawal from the Medical College of Georgia;
     

  2. Employee resignation, termination, retirement, or written request to close account
     

  3. A consecutive 180-day period in which no customer transaction takes place within the account. (See Section VIII.)

II. ELIGIBILITY

An MCG ExpressCard account may be opened by anyone with a valid picture ID card that is employed or enrolled at the Medical College of Georgia or MCG Health, Inc. Frequent visitors or guests may request a non-photo card/account.

III. MINIMUM DEPOSIT

The suggested minimum initial deposit for the MCG ExpressCard account is $25. Once the account is established, the minimum amount of an additional deposit is $1.

IV. GENERAL OPERATION

Upon receipt of the customer’s initial deposit and signed application, the MCG ExpressCard office will establish an electronic balance in the customer’s name and validate the customer’s ID/MCG ExpressCard for use in the electronic card readers. The customer may then use their pre-deposited funds for purchases, during location business hours, wherever the MCG ExpressCard is accepted. Purchases attempted against insufficient funds will not normally be allowed. Customer accepts responsibility for any offline and/or signature transaction made to their account against insufficient funds.

Purchases cannot be allowed with a malfunctioning card. If the card malfunctions, it should be brought to the MCG Express Office or to MCG Public Safety Badging office. It will be replaced, usually at no charge, once card personnel verify that the malfunction is due to a faulty card.

V. LOST OR DAMAGED CARD REPLACEMENT

Lost or damaged cards will be replaced upon the customer’s request at the MCG Public Safety/ID Badging office. Replacements will be made in accordance with current policies regarding card replacement and, in most cases, the customer will be charged a replacement fee of $10. The customer may use his MCG ExpressCard account funds to purchase the replacement card(s).

VI. LIABILITY FOR ILLEGITIMATE USE OF AN ACCOUNT

The customer is responsible for safeguarding his MCG ExpressCard/ID. Should the card become lost or misplaced, whether accidentally or by theft, the MCG ExpressCard office and the Medical College of Georgia assume no responsibility for illegitimate withdrawals from the customer’s account prior to his notification in person or by phone to the MCG ExpressCard Office (706-721-9939) or the MCG Public Safety/ID Badging office (706-721-8113) or online at www.edu/express. Upon notification, the account will be rendered unusable until the rightful owner has the card/account restored. Lost or misplaced I.D. cards should be reported immediately to prevent misuse at unattended locations. With proper notification, account funds are secure until the card is recovered or replaced. Only the MCG ExpressCard office or the MCG Public Safety/ID Badging office can reactivate a card after it is reported lost or stolen.

VII. REFUNDS

MCG ExpressCard account balances will be carried forward until either a student graduates, is dismissed, or withdraws from school, or an employee resigns, retires, is terminated, or formally closes the account. Refunds of $25 or more are normally made only by mailed check, within 20 calendar days after the customer’s request is received in writing at the MCG ExpressCard office. Refunds are mailed to the address provided at the time refund is requested. Partial refunds are not made. No cash withdrawals permitted at any time.

VIII. INACTIVE ACCOUNTS

To protect the customer, the MCG ExpressCard office reserves the right to temporarily deactivate any account not used during a consecutive 90-day period. When the customer wishes to resume using the MCG ExpressCard account, it may be restored by contacting the MCG ExpressCard Office by phone or in person.

Should 180 consecutive days pass in which no customer transaction takes place in these accounts, and if there is a remaining balance of $25 or more, the MCG ExpressCard office may attempt to contact the customer and inquire if the account should remain open. If the customer replies to this inquiry within 30 days, the MCG ExpressCard office will either leave the account open, or close the account and refund any remaining balance of $25 or more, as instructed by the customer in writing. If the customer has graduated, withdrawn, terminated, retired, or resigned and fails to respond within 30 days, then the balance becomes the property of the MCG ExpressCard office. Should any account balance be less than $25, and if 180 consecutive days pass with no customer transaction, or if the customer has graduated, withdrawn, terminated, retired, or resigned, then the balance becomes the property of the MCG ExpressCard office without prior notice to the customer.

IX. FUTURE CHANGES IN TERMS

Future changes in terms and conditions regulating the use of this card will be made in writing and will apply to all cards in circulation and in use at that time. This written notice will supersede the terms and conditions in effect at the time the card was acquired.

Customers may elect to cancel their account with a written request to the MCG ExpressCard office. Any such request must be received within 30 days from the notification date of the terms and conditions change.
