Student Center Reservation Request Form

Please complete the entire form to request a reservation.  An email reply will be provided within 48 hours.  Most rentals require a fee for use.  The total fee and any additional approvals will be included in the email response.

**A fee may be assessed for Environmental Services Clean-up or Set-up and contacting these departments is the responsibility of the user group.   

**Set-up of the room is not included with reservation.
