   AUTHORIZATION

INTRODUCTION

All persons who use the therapeutic sources of radionuclides at any of the hospitals named in the preamble of this text must either (1) be authorized by the Subcommittee, (2) work under the direct supervision of an individual who is authorized, or (3) be a radiation therapy resident approved by the Medical Director, Georgia Radiation Therapy Center.

 PREREQUISITES FOR AUTHORIZATION

The applicant must:

1. be a physician licensed to practice medicine in Georgia
2. have adequate training and experience for the proposed uses, and
3. have the use of adequate facilities to control levels of external radiation and surface contamination to the general public and to occupationally exposed individuals.

PROTOCOL

1. Applications for authorization for therapeutic uses may be obtained from the Radiation Safety Office. A preceptor statement must be included with the application.

2. The completed application must be sent to the Radiation Safety Office for review. After review, recommendations will be made to the Subcommittee for appropriate action.

3. In accepting an authorization, the applicant agrees to make radioactive material records available for inspection by the Radiation Safety Office or the Georgia Department of Natural Resources at all reasonable times .
