Required Components of a Promotion Packet
- Completed by faculty member
- Curriculum Vitae (in MCG format)
- Educator’s Portfolio
- Completed or requested by department (chair, designee)
- Promotion Recommendation Form Guidelines
- Promotion Recommendation Form Addendum Guidelines
- Outstanding Service Form
- Letters of recommendation (internal and external) should be requested in writing by the department chair or departmental Promotion and Tenure committee chair. The request should include:
a. Candidate's CV
b. Educator's Portfolio if the candidate is requesting promotion substantially because of education contributions
c. FADPT criteria for that person's track
School of Medicine guidelines by track / pathway for that candidate's pathway
d. Describe your relationship with the candidate
- Completed by faculty member and/or department
- Education, Service (clinical, committees, etc.), Research
Activities Form (annual data for last 5 years)
